Either he stays fit and keeps playing to a high level and sees out the contract (not unusual for a defender to play until mid 30s) or he retires if he feels like it’s time. Makes no odds really, this is just a way of showing his commitment to the club, and vice versa, and that he wants to finish his career here. Some are still able to maintain a high standard at 38, some are done by 33. Point being, you can have a reasonable idea what level of player a 26 year old will be when they're 31, and even then you don't see five year deals being handed out at this level.
